WHAT INFORMATION IS NEEDED?

In order to ensure that you receive adequate compensation for your injuries, you should make sure to retain all of the documentation from your accident. Organize this information in folders or notebooks, and bring copies of everything to your initial consultation.

Examples of documentation you will need to keep:

  • Copy of the Police Report
  • Complete description of what happened
  • Your insurance information:
    • Insurance carrier name, address and telephone number
    • Policy/claim number
    • Insurance cards
    • Local adjustor/agent
    • Any correspondence with the insurance agent/adjustor
  • Negligent party's insurance information:
    • Insurance carrier name & address
    • Insurance policy/claim number
    • Local adjustor/agent
  • Witness contact information
  • Copies of receipts and/or bills for expenses:
    • Wrecker or towing
    • Costs of storage
    • Costs of repairs
    • Costs of rental car
    • Damage to other personal items
    • Mileage to and from repair shop or medical facility
  • Copies of documentation for medical treatment:
    • Medical billings
    • Medical records
    • Doctor's notes
    • Correspondence with medical facility
  • Insurance explanation of benefits
  • Copies of documentation for wage/profit loss:
    • Name and address of employer
    • Affidavit of wage verification
    • Documentation of salary before taxes (hourly or weekly)
    • Documentation of days absent due to accident
    • Doctor's work excuses or restrictions
